Violations marked as priority contribute directly to the elimination, prevention or reduction in the hazards associated with foodborne illness. Priority violations include prevention of contamination, cooking, reheating, cooling and handwashing.

Core violations include items that usually relate to general sanitation, operational controls, equipment design or general maintenance.
210 U.S. 412 East, Siloam Springs
Priority violations: No sanitizer was detected with test strips in the automatic dishwasher due to faulty dispensing. The potato soup was not cooled properly.
Priority foundation violations: No soap was available at the bar handwashing sink. No paper towels were available at the bar or servers handwashing sinks. Employee personal foods were stored above facility food in the servers cooler.
Core violations: Floor tiles were missing or loose around the bottom of the wall in the dish room. There were holes in sheetrock where tiles were missing. There were loose and broken tiles and a loose threshold at the meat walk-in cooler. The floor in the dish room was no longer in good condition with missing or loose floor tiles, missing grout and water was wearing away the concrete around the floor drain. Wooden 4x4 posts were holding up the walk-in freezer fan unit. The mop sink room floor was missing concrete in places, allowing for standing water.
120 U.S. 412 East, Siloam Springs
Priority violations: Foods in the small black cooler in the front were at 56 degrees. Foods in the two-door upright cooler were not at 41 degrees or below. A spray bottle previously used for another chemical was now labeled as bleach water.
Priority foundation violations: Employees were washing and sanitizing items manually and then placing the items in the dish machine. The dish machine was not reaching the correct temperatures to sanitize correctly.
Core violations: The thermometer in the front two-door reach-in appeared to be faulty with a reading of 46 degrees, yet the foods were at 39 degrees. Employees working with open food were not wearing effective hair restraints. An employee working with open food was wearing jewelry on her hands. Areas where the floor had been repaired using pebble type tile never had the grout around the pebbles installed, exposing the backing of the tile. The floor was not smooth and easily cleanable and allowed for standing water and food debris to get between the pebbles, repeat violation.

Priority foundation violations: Fried chicken was being kept under time control, but there was no labeling/log showing when the fried chicken was to be disposed of.
Core violations: Utensils for hot food were being stored in warm water at 111 degrees. The facility lacked quat test strips. The outside of the knife block had an accumulation of food residue. The shelves around the three-compartment sink had an accumulation of food residue.
Priority foundation violations: A cutting board and knife were present in the handwash sink. No test strips were available.
Core violations: The facility had no certified food manager documentation available. Uncovered employee beverages were located throughout the prep area. No permit was posted.
Core violations: The facility had no certified food manager documentation available. Fabric bags of rice were being stored directly on the floor of the dry storage area.
Priority violations: A tray of prepared salads was not covered in the upright cooler. In the front cooler, cheese was at 49 and 50 degrees and chicken at 49 degrees.
Core violations: Multiple flying insects were in kitchen area. The large garage doors were open in the dining area. Two waste receptacles outside the back door were not closed.

Core violations: The surfaces of the cutting boards had deep scratching, scoring and chipping, creating a surface that could not be effectively cleaned or sanitized.
Core violations: Uncovered employee beverages were located throughout the prep area. The women's restroom lacked handwash signage. Cardboard boxes and paper bags of food items were being stored directly on the floor of the walk-in cooler and dry storage area. An unhandled bowl was being used to scoop beans from a refrigerated bulk container.
123 N. Broadway St., Siloam Springs
Priority violations: The sanitizer concentration in the three-compartment sink was not strong enough. Butter in the bottom of the bakery cooler was not at 41 degrees or below. The half-and-half in the self-serve pitcher was at 76 degrees.
Priority foundation violations: Food contact surfaces and utensils were not being cleaned at least every four hours as required for facilities using time/temperature control. The potato salad in the reach-in prep table exceeded the seven day use-by date.
Core violations: No handwashing signage was at the handwash sink. A steam pitcher and whisk were in the handwashing sink. The sanitizer wiping cloth solution for the steam wand was not strong enough. The gasket and inner door panel were falling off in the milk reach-in cooler, repeat violation.

Priority foundation violations: The facility had no written time documentation that the fried chicken was being held under time control.
Core violations: Multiple containers of food were not labeled with what foods they contained. Bags of onions were stored directly on the kitchen floor. The rice scoop was stored in water at 86 degrees.
817 E. Main St., Siloam Springs
Priority foundation violations: Utensils were in the handwashing sink. Not all foods were date-marked as needed.
Core violations: At least one employee who has supervisory and management responsibility and the authority to direct and control food preparation and service shall be a certified food protection manager who has shown proficiency of required information through passing a test that is part of an accredited program. Containers of food were not covered during storage in the refrigerator. The back store room was carpeted.
Core violations: The facility had no certified food manager documentation available.
920 S. Carl St., Bldg. 4, Siloam Springs
Core violations: The facility was using low splash bleach for sanitizing food contact surfaces. The thermometer in the refrigerator No. 1 was not working properly, with the indicator showing low battery.
Priority violations: Meatballs were being held at 108 degrees, chili at 110 degrees and potato soup at 114 degrees in hot-holding unit.

Core violations: The facility had no certified food manager documentation available.
2005 S. Promenade Blvd., Suite 100, Rogers
Priority foundation violations: The spring was broken on the water fixture for the three-compartment sink causing the hose to hang below rim level of the sink.
Core violations: An accumulation of food debris was inside the small cold-holding prep table unit near the food prep line. An accumulation of dust, dirt and food debris was beneath the prep table near the fryer and beneath the shelving units and hot-holding unit in the back of the kitchen. The permit was not posted in customer view.

Priority foundation violations: The handwash sink nearest the walk-in cooler was filled with knives.
Core violations: The certified food manager documentation available was expired. Uncovered employee beverages were located throughout the prep area. Multiple main line workers lacked hair restraints.
2005 S. Promenade Blvd., Suite 140, Rogers
Priority violations: Strawberry boba was being held at 59 degrees in the cold-holding unit.
Priority foundation violations: A bottle of cleaner was not labeled with contents of the container.
2003 S.W. Regional Airport Blvd., Suite 15, Bentonville
Priority violations: The fajita beef was at 106 degrees. The salsas were at 45 degrees.
Core violations: Medicines were stored with seasonings and dry goods. The queso bucket was stored in a way that did not facilitate heat dispersion. Temps were taken at two separate times and were at 81 degrees at 1:40 p.m. and 80 degrees at 2:00 p.m. Beans were stored without a lid. The facility was using a condiment bowl as a scoop. Knives were being stored between the drain table and wall, an Arkansas Department of Health unapproved location. A rack by the cooler and a pot rack above the counter had buildups of grease and debris.
Core violations: The facility had no certified food manager documentation available.
